FIELD MANUAL — HARDEST DIFFICULTY
REQUIEM INSANITY — EVERYTHING THAT CHANGES
Insanity is Resident Evil Requiem's hardest difficulty, and it does not just tune the numbers — it reroutes your entire mental map. All five safe codes change, puzzle solutions change, item and antique coin locations move, enemies hit harder, and Grace's save system reverts to the old-school typewriter and ink ribbon discipline. This guide lines up every verified change against Normal, side by side.
First, the mindset: on Casual and Standard, Requiem forgives a wasted bullet and a forgotten save. On Insanity, the parts of the game that actually cost you a playthrough are all deliberately sharpened: saves, safes, item memory. Here is the full accounting.
SECTION 01
01Normal vs Insanity — the Side-by-Side Table
Everything below is verified against IGN, Polygon, GodisaGeek and our own guides hub.
| Feature | Casual / Standard | Insanity |
|---|---|---|
| Safe #1 — Bar & Lounge (Care Center, West Wing) | L 10 · R 80 · L 30 | R 20 · L 50 · R 30 |
| Safe #2 — Examination Room (Care Center) | R 30 · L 10 · R 50 | L 50 · R 30 · L 90 |
| Safe #3 — Furnace (Care Center Basement) | R 60 · L 40 · R 80 | L 70 · R 50 · L 20 |
| Safe #4 — Sterilization Chamber (ARK) | R 10 · L 90 · R 20 | L 80 · R 50 · L 0 |
| Safe #5 — Monitor Control Room (ARK) | R 50 · L 60 · R 80* | R 20 · L 0 · R 70 |
| Non-safe puzzle locks (STARS briefcase, Chairman's Office, Lead Researcher's Office) | Fixed | Same — never change |
| Puzzle solutions generally | One set | Changed |
| Item locations | Standard placements | Relocated |
| Antique Coin spawns | 22 on the standard route (PowerPyx) | Differ from the standard route |
| Enemies | Standard threat curve | Tougher enemy mixes, hit harder |
| Saves | Standard manual saves | Typewriter / ink ribbon save system for Grace |
| Files revealing safe codes | In place | Same places — the code files don't move |
* The Safe #5 Normal code is the one disputed number in the game's data: IGN prints R 50 · L 60 · R 80, while GamerLume, Beebom and NeonLightsMedia publish the mirrored L 50 · R 60 · L 80. Try IGN's first, fall back to the mirror. The Insanity column has no such asterisk — every source agrees on it. Full dial directions and rewards on the safe codes page.
SECTION 02
02How to Unlock Insanity
Finish Resident Evil Requiem once on any difficulty and Insanity unlocks — no DLC, no special conditions. Roll the credits, return to the main menu, and the hardest difficulty in the game is waiting there for your new run (IGN, Cheats, Secrets, and Easter Eggs). Insanity ships in the base game, so a standard copy is all it takes — and if you want to see the mode before committing, the complete Insanity walkthrough on our Videos page runs it start to finish, The Commander boss included.
SECTION 03
03What Actually Changes — Section by Section
Enemy Pressure
Insanity runs tougher enemy mixes and enemies that hit harder (GodisaGeek, IGN). The practical consequence is not just shorter health bars — it is that the habits that were optional on Normal become load-bearing. Healing before a boss door instead of after. Leading with the magnum on an unaware enemy instead of trading shots. Our beginner's survival tips cover the full set; on Insanity, treat all eight as mandatory.
Saves — the Ink Ribbon Problem
On Insanity, Grace's save system reverts to typewriters and ink ribbons — the classic Resident Evil economy where saving itself costs a resource (the same system Classic modes bring back). Every save is a decision. The habit that carries over from completion runs applies double here. Before you leave any area for good, decide whether this is a moment worth an ink ribbon — the route does not loop back.
Items, Coins and the Changed Map
Item locations relocate on Insanity, and Antique Coin spawns differ from the standard route — so a first-run mental map is worse than useless; it is confidently wrong. The consolation prize: Antique Coins carry no trophy requirement, so on an Insanity run a missed coin costs you upgrades, not completion. Spend your attention on the files and statues instead, and let the coin hunt be opportunistic.
Safe Codes
All five dial combinations swap, as tabled above. Two things do not change: the in-game files that reveal the codes stay put (IGN), and the symbol puzzle locks never change on any difficulty. So the Insanity loop is: distrust every dial, trust every Moon-Sun-Star puzzle, and the code files are exactly where you remember them.
One more quirk worth knowing before you plan around safes: the Waiting Room's sixth safe on Rhodes Hill is already open when you arrive and holds 2 antique coins on Standard — and it does not count toward No Safe is Safe. That trophy tracks the five locked safes, whose Insanity codes are printed above.
SECTION 04
04Insanity Survival Rules
The mode's difficulty curve is the same campaign with sharper teeth, so the survival fundamentals do the heavy lifting:
- Orange health is one hit from death. A strong enemy's swing at orange usually kills outright — heal before the boss door, not after (Polygon).
- "Cleared" rooms refill, and sloppy kills come back. Zombies killed without a headshot return as faster, tougher blisterheads (Polygon, IGN). On Insanity, every non-headshot kill is a loan against your future.
- Hemolytic Injectors are your stealth kill. On an unaware enemy, one silent hit, no corpse to reanimate — IGN ranks crafting it (60 blood + 1 scrap) above stockpiling spare ammo.
- Lead with the REQUIEM magnum. Grace starts with the best gun in the game: an unaware stalker drops to one headshot; an alerted one eats 3-5 rounds (IGN). On Insanity, that difference is the difference.
- Reload the save, not the magazine. A wasted early burst costs more than a reload; tune aim assist in Options if aiming keeps failing (Polygon).
- Some published routes assume no infinite-ammo shortcuts. If you follow a walkthrough, check which economy it was built for.
SECTION 05
05Rewards for Clearing Insanity
Clearing Insanity grants the Remarkable Agent achievement and a 40,000 CP reward (Deltia's Gaming, GamesHedge). The CP feeds the game's bonus content system, where points trade for extras such as infinite ammo — a useful safety net to carry into whatever run comes next.
SECTION 06
06Is Insanity Worth It?
The headline reward is 40,000 CP and the Remarkable Agent achievement (see the section above), but the bigger payoff is the run itself — the game's survival-horror core with the safety rails removed, and the typewriter economy making every forward step a commitment. If you are chasing completion trophies instead, note that the coin set demands nothing on any difficulty, and the collectible sets that carry across playthroughs don't care which difficulty you cleared them on. Our collectibles guide separates the trophy-essential sets from the optional ones.
QUICK ANSWERS
07Insanity FAQ
What changes on Insanity difficulty in Resident Evil Requiem?
All five safe codes, the puzzle solutions, item locations and Antique Coin spawns change; enemies hit harder in tougher mixes; and Grace's saves revert to the typewriter and ink ribbon system. The non-safe symbol locks and the code-revealing files stay put (IGN, GodisaGeek).
Are the safe codes different on Insanity?
Yes — all five, not just some. Safe #1 R 20 · L 50 · R 30, #2 L 50 · R 30 · L 90, #3 L 70 · R 50 · L 20, #4 L 80 · R 50 · L 0, #5 R 20 · L 0 · R 70. Dial directions and rewards are on our safe codes page.
Do I need infinite ammo for Insanity?
No — and note that some published Insanity routes are built around the assumption that you have no infinite-ammo shortcuts. The mode is beatable on the standard economy; the walkthrough on our Videos page is proof.
Do Antique Coins matter on Insanity?
For trophies, no — the coin set has no trophy requirement on any difficulty. Coins buy permanent Parlor upgrades, and Insanity's coin spawns differ from Normal, so treat them as a bonus hunt, not a checklist.
